Responsibilities
- Develop, implement, and evaluate care management program lines within a health plan
- Promote continuity of care and cost-effective management through whole-person in-patient and out-patient care
- Coordinate care integration, case management, utilization review, and comprehensive care planning
- Collaborate with multidisciplinary teams to create and monitor individualized care plans
- Document care plans, outcomes, and progress; ensure regulatory compliance and quality standards
- Educate members and families about available resources and services; facilitate appropriate referrals and transitions

Requirements
- Current, active RN license in California
- CPR/BLS certification
- Strong communication and collaboration skills
- Ability to work in a fast-paced health plan care management environment
- Preferred: experience in case management or health plan care coordination
